If you have Primecare insurance coverage you must contact them to select your PCP Before your appointment. If this is not done, you may be responsible for the charges due at that appointment. This also applies if your insurance plan is an HMO, such as Anthem

We understand that unexpected circumstances arise at times that will make it necessary for you to cancel an appointment with your Provider. If you are unable to keep your appointment, please call 355-3512 to cancel or reschedule, 24 hours in advance. We may be able to schedule someone else in need of an appointment in your place if we have advance notice.
